The report found that the castle was not anchored according to manufacturer instructions, and weather conditions, including strong winds, had not been properly considered. The investigation also pointed to a lack of clear training or qualifications for those operating the equipment, and no formal safety checks were in place prior to use.
These findings serve as a heartbreaking reminder of the importance of risk assessments, supervision, and adherence to manufacturer guidelines - especially when it comes to temporary or inflatable playground equipment.
